Sildenafil, Kamagra, and nitrates: why combining them is life-threatening

This article is intended for informational purposes only and does not replace medical advice. If in doubt about drug interactions, contact your doctor or pharmacist immediately.

In the United States, approximately 5,5 million men use nitrates daily for angina pectoris ( Circulation, AHA , 2011). Some of these men also have erectile dysfunction and consider sildenafil or Kamagra. That is a potentially deadly combination.

Sildenafil (the active ingredient in Viagra and Kamagra) must never be combined with nitrates. Not over a few hours, not in a "lower dosage," and not "when you are feeling well." The interaction is predictable, dose-dependent, and can be fatal. In this article, you will read exactly why, which substances are dangerous, and what to do if things do go wrong.

Warning: absolutely do not combine
  • Sildenafil/Kamagra + nitrates = life-threatening drop in blood pressure
  • Wait at least 24 hours between sildenafil and any nitrate (FDA, 2017)
  • Poppers (amyl nitrite) are also nitrates and just as dangerous.
  • 69 deaths reported to the FDA within 5 months of market introduction, of which 12 were related to nitrate use (AHA Circulation, 1999)

What happens in your body when you combine both substances?

Both sildenafil and nitrates lower blood pressure via the same biochemical pathway: the nitric oxide-cGMP system ( StatPearls, PDE5 Inhibitors , 2025). When combined, they exponentially amplify each other's effect. The result? A drop in blood pressure that your body cannot compensate for.

This is how it works step by step. Nitrates are converted into nitric oxide (NO) in your body. This NO stimulates the enzyme guanylate cyclase, which produces cGMP. cGMP relaxes the smooth muscle cells in blood vessels, causing them to dilate and blood pressure to drop.

Sildenafil inhibits the enzyme PDE5, which normally breaks down cGMP. By blocking PDE5, cGMP remains active longer. That is the desired effect during an erection: increased blood flow to the erectile tissue.

But if you take nitrates at the same time, a double effect occurs. The nitrates pump extra cGMP into the system, while sildenafil prevents it from being broken down. The result: massive, uncontrolled vasodilation throughout the body. Your blood pressure plummets.

The core problem: Nitrates increase cGMP production. Sildenafil prevents the breakdown of cGMP. Together, they cause an uncontrolled accumulation of cGMP, leading to an extreme and potentially fatal drop in blood pressure.

How much does blood pressure drop with combined use?

In a double-blind crossover study in 34 men with coronary heart disease, sildenafil was found to halve the maximum tolerated nitroglycerin dose: from 160 to 80 micrograms per minute ( PubMed , 2007). Only 25% of participants tolerated the maximum nitroglycerin dose after sildenafil, compared to 59% with placebo.

A second study made it even clearer. In healthy men who received sildenafil 100 mg followed by a sublingual nitroglycerin tablet, sitting systolic blood pressure decreased by 28,7 mmHg, compared to 16,6 mmHg with placebo. That is an additional decrease of over 12 mmHg ( Webb et al., British Journal of Clinical Pharmacology , 2008). The difference was even greater for standing blood pressure.

The FDA package leaflet states that sildenafil alone lowers systolic blood pressure by 8-10 mmHg ( FDA Prescribing Information , 2017). Combine that with a nitrate, and you enter a zone where organs are insufficiently perfused. In angina patients, the additional drop in blood pressure remained measurable up to 8 hours after sildenafil intake.

Blood pressure reduction: sildenafil vs. nitrates vs. combination Sildenafil alone causes a systolic drop of 8-10 mmHg, nitrates alone 20-30 mmHg, and the combination can cause more than 40 mmHg. Systolic blood pressure drop (mmHg) indicative, individual variation is high Sildenafil alone 8-10mmHg Nitrate only 20-30mmHg Combination >40 mmHg The combination can lead to unconsciousness, shock, or death. Which medicines are nitrates (and which are often forgotten)?

The FDA classifies all organic nitrates as absolutely contraindicated with sildenafil ( FDA Prescribing Information , 2017). Yet, many men do not know which medications contain nitrates. And some nitrates are not recognizable as such.

Substance name Brand names (examples) Shape Risk with sildenafil
Nitroglycerin Nitro Pohl, Nitrolingual, Deponit Spray, patch, tablet Absolutely forbidden
Isosorbide mononitrate Promocard, Monoket Tablet Absolutely forbidden
Isosorbide dinitrate Cedocard, Isordil Tablet, spray Absolutely forbidden
Amyl nitrite / butyl nitrite poppers Inhalation fluid Absolutely forbidden
Nicorandil Dancer, Ikorel Tablet Absolutely forbidden
Molsidomine Corvaton, Molsidolat Tablet Absolutely forbidden

Note: nicorandil and molsidomine are not strictly speaking "nitrates", but they act via the same nitric oxide mechanism. They are therefore also absolutely contraindicated with sildenafil or Kamagra ( Prescriber, Wiley , 2011).

Choose Sildenafil dosage: 25, 50 or 100 mg (start safely)

How dangerous are poppers in combination with Kamagra or Viagra?

Poppers (amyl nitrite, butyl nitrite, isobutyl nitrite) are recreational nitrates that are inhaled for a short-lived "rush" sensation. Research among young MSM (men who have sex with men) with HIV found that 73% had combined poppers with erectile dysfunction medication ( PMC , 2024). An extremely risky combination.

Poppers dilate blood vessels just like medical nitrates. If you combine them with sildenafil or Kamagra, you get the same dangerous dual effect on the cGMP system. The result: an extreme drop in blood pressure, dizziness, unconsciousness, and in the worst case, cardiac arrest.

Many users think that poppers are "harmless" because they are available over the counter. However, the FDA package insert for Viagra explicitly lists amyl nitrite as contraindicated. There is no safe way to combine poppers and sildenafil, regardless of the dose or the order of administration.

Common mistake: "I only take poppers an hour after taking Viagra, then it is safe." This is incorrect. Sildenafil remains active in your body for up to 18 hours. Poppers within that period are dangerous.

How long should you wait between sildenafil and nitrates?

The FDA prescribes a minimum waiting period of 24 hours between sildenafil and any nitrate ( FDA Prescribing Information , 2017). Nitrates may not be administered until after 24 hours, and only under medical supervision. But what about the other PDE5 inhibitors?

PDE5 inhibitor half-life Waiting time until nitrate Source
Sildenafil (Viagra, Kamagra) 3-5 hours Minimum 24 hours FDA
vardenafil (Levitra) 4-5 hours Minimum 24 hours FDA
Tadalafil (Cialis) 17,5 hour Minimum 48 hours FDA
avanafil (Spedra) 5 hour Minimum 12 hours EMA

Do you use nitrates daily for angina pectoris? If so, you may not use any PDE5 inhibitor, regardless of the withdrawal period. The contraindication applies as long as you have nitrates in your treatment plan. Do you still wish to use sildenafil? Then discuss with your cardiologist whether you can be switched to a non-nitrate alternative such as ranolazine ( PMC , 2014).

What do you do in an emergency after combined use?

In the event of a severe hypotensive reaction following combined use of sildenafil and nitrates, rapid medical intervention is necessary. The AHA emphasizes that physicians must be prepared for fluid resuscitation and alpha-adrenergic vasopressors ( Circulation, AHA , 2011).

What you can do yourself (while waiting for help)

  • Beautiful 112 immediately
  • Lie flat with legs up (Trendelenburg position)
  • No alcohol drink, this lowers blood pressure further
  • No extra medication to take
  • Tell the ambulance staff exactly which substances you have taken and when

What doctors do in the emergency room

  • Intravenous fluids to restore blood volume
  • Alpha-adrenergic vasopressors (such as phenylephrine) to raise blood pressure
  • Monitoring of heart rhythm, blood pressure and oxygen saturation
  • No added nitrates do not administer, even if the patient has chest pain
Critical point for doctors: Nitrates should not be administered to a patient with chest pain who has used sildenafil. This applies for at least 24 hours after sildenafil intake. Use alpha-adrenergic vasopressors if blood pressure treatment is required.

How many men are at risk of this interaction?

A large American study using electronic medical records found that 1 to 4% of men with a PDE5 inhibitor prescription also possessed nitrates ( Journal of Sexual Medicine , 2021). Among more than 168.000 PDE5 inhibitor users, 3.167 men had periods of simultaneous possession of both medications.

A Danish national study covering the period 2000–2018 identified 42.073 men with ischemic heart disease who were using nitrates and were subsequently prescribed a PDE5 inhibitor ( ACC Journal Scan , 2022). The prescribing rate rose from 0,9 per 100 persons per year in 2000 to 19,5 in 2018.

Alarming: of the patients with documented concomitant possession, only 53% were warned about the dangerous interaction. Almost half received no warning from their doctor or pharmacist.

What alternatives do you have if you use nitrates?

If you need daily nitrates for angina pectoris, you cannot use sildenafil or Kamagra. But that does not mean there are no options. The Princeton IV Consensus Guidelines recommend looking at alternatives together with your cardiologist ( Journal of Sexual Medicine, Princeton IV , 2024).

Anti-anginal alternatives (safe with sildenafil)

  • Ranolazine (Ranexa): works via sodium channels, no interaction with PDE5 inhibitors
  • Beta blockers (metoprolol, bisoprolol): safe to combine, but monitor blood pressure
  • Calcium channel blockers (amlodipine, diltiazem): safe, caution with blood pressure
  • Ivabradine (Procoralan): lowers heart rate without blood pressure effect

Non-pharmacological ED options (without interaction risk)

  • vacuum pump: mechanical, no drug interaction
  • Alprostadil injection (Caverject): local action, no systemic interaction with nitrates
  • MUSE Photo Studio (intraurethral alprostadil): local application
  • Penile prosthesis: surgical option in case of failure of other treatments

Important: Never switch from nitrates to another medication on your own. Your cardiologist must assess whether you can safely stop taking nitrates and what a suitable alternative is for your specific situation.

Frequently asked questions

How long should I wait between nitrates and sildenafil?

After taking sildenafil or Kamagra, you must wait at least 24 hours before using nitrates. The same applies in reverse. For long-acting nitrates such as isosorbide mononitrate, some guidelines recommend 48 hours. For tadalafil (Cialis), the waiting period is 48 hours due to the longer duration of action ( FDA , 2017).

Are poppers also dangerous in combination with Viagra or Kamagra?

Yes, poppers (amyl nitrite, butyl nitrite) are recreational nitrates and just as dangerous as medical nitrates. The combination can cause a severe drop in blood pressure, unconsciousness, and death. Poppers are explicitly listed as contraindicated in the FDA package insert.

What should I do if I have accidentally taken both substances?

Call 112 immediately or go to the emergency room. Lie flat with your legs elevated to support your blood pressure. Do not drink alcohol or take any other medication. In the hospital, doctors will treat the low blood pressure with intravenous fluids and, if necessary, vasopressors.

Can I use sildenafil if I take blood pressure medication (excluding nitrates)?

Sildenafil is not absolutely contraindicated with non-nitrate blood pressure lowering agents, but caution is advised. With alpha-blockers (tamsulosine, doxazosine), it is recommended to start with 25 mg. Always consult your doctor so that your blood pressure can be monitored.

Are there safe alternatives to nitrates if I want to use sildenafil?

Yes, there are anti-anginal medications that are not nitrates, such as ranolazine, beta-blockers, and calcium channel blockers. These are not contraindicated with sildenafil. Please note: nicorandil and molsidomine are NO donors and are also contraindicated. Always discuss alternatives with your cardiologist.

Sildenafil side effects: complete overview (mild to severe)

Conclusion: zero tolerance for this combination

If you do not use nitrates and want to buy Kamagra , start with Kamagra 100mg . Also check the safety checklist.

The message is unambiguous: sildenafil, Kamagra, and all other PDE5 inhibitors must never be combined with nitrates. No exceptions, no "safe" dosages, no timing tricks.

What you need to remember:

  1. Nitrates + sildenafil = life-threatening drop in blood pressure via cGMP stacking
  2. Poppers are also nitrates and just as dangerous as medical nitrates
  3. Wait at least 24 hours after sildenafil before using nitrates (48 hours for tadalafil)
  4. Do you use nitrates daily? Discuss alternatives with your cardiologist.
  5. In case of emergency: call 112 and lie flat with your legs raised
